Pet Hotel Registration Form
Please use this form after scheduling an appointment with our staff to register your pet for the service listed above. You may submit this form online or fill out the information requested below and fax to 859-268-7872.
All pets boarded or groomed at Sheabel Pet Care Center are required to be current on all their vaccinations, including (for dogs): DHPPC, Rabies, Bordetella, Leptospirosis and a Fecal Exam. Cats are required to be up to date on Rabies, Feline Distemper, Feline Leukemia, and Fecal Exam. We require information from your veterinarian providing the dates these vaccines were given. Please bring this information when your Pet checks into Sheabel Pet Hotel. Any animal found to be not up to date will be given these vaccines at the owner’s expense. Any pet found to be contaminated with fleas or ticks will also be treated at the owner’s expense.

By submitting this form, you do hereby grant permission to Sheabel Pet Care Center to administer any necessary veterinary treatment to your pet as required, as well as any emergency medical procedures that may be needed. You agree not to hold this establishment responsible for any illness of or injury to your pet, as sometimes boarding and / or grooming can expose a hidden problem or aggravate a current one.
