Please use this form after scheduling an appointment with our staff to register your pet for the service listed above.  You may submit this form online or fill out the information requested below and fax to 859-268-7872.

All pets boarded or groomed at Sheabel Pet Care Center are required to be current on all their vaccinations, including (for dogs): DHPPC, Rabies, Bordetella, and a Fecal Exam. Cats are required to be up to date on Rabies, Feline Distemper, Feline Leukemia, and Fecal Exam. Any animal found to be not up to date will be given these vaccines at the owner’s expense. Any pet found to be contaminated with fleas or ticks will also be treated at the owner’s expense.
